I believe from everything I've read elsewhere that they did indeed ship OS 0.1 on 4 EPROMS with a free upgrade to 1 IC later when it became available. Now you've mentioned it, I do remember that. As promised, attached is a zip with the HostFS stuff. Note this is a RISCOS zip so the files should be correctly filetyped but they can be viewed on a PC as well (although the text files will be messed up due to CR/LF differences). Somewhere I've got a small program which patches ADFS to use different SCSI IDs for different drive numbers. Let me know if you want a copy and I'll try and find it.
I think I also have a version of ADFS that's been patched to handle 512 byte sectors.

I know this is a bit late but I recently had a similar problem with self destructing (Wabash) floppies which clogged one of the drive heads and normal cleaning discs and tools didn't fix it. I eventually fixed the clogged head by placing a strip of paper soaked in isopropyl alcohol between the heads...
