Without sounding like a broken record, you need to patch ADFS.

I think there's an updated ADFSUtils in the ROOL System Resources to patch it properly, going by Phlamethrowers last comment.

Looks like it's causing the OS to crash going by the Abort address, I'll have to reproduce the issue and do some debugging to find out why. As its occurring the second you load it, that limits it down a bit. To confirm which Module is triggering the crash can you see how far you get before it crashe...

Everything in the Tasks folder is run post desktop, so move everything in there to another folder outside of Tasks and see if you then get a working machine. Assuming you do, you can move things back until it breaks.

I believe LK4 was very short lived and only on the 540 and R-series so there's probably not been much demand for it. I guess it was replaced with JTAG from ARMv3.

If you leave it for 30 mins, do you get an error?

It's either a disc timeout, or something is loading post desktop that's getting stuck. Look at post desktop tasks loading under !Boot.Choices.Boot

Well that does seem to allow it to set MODE 13, although it identifies as 640x480 at 64 colours (256 with tints) in BASIC. You should use OS_CheckModeValid to check if a screen mode is available for use. It will return a suggested mode if it's not valid. MODE 13 is perfectly valid on a Pi, provided you disable GPU mode change support by creating !Boot.Loader.CMDLINE/TXT with the following contents: disable_mode_changes

I've found that Sandisk cards seem to work pretty well in IDE mode, but Apacer's (even their industrial cards) will identify, only to throw read-write errors after some indeterminate period of time. If that issue only occurs on RISC OS, it will be down to the driver timing out DRQ as per the origin...
