Omniflop partly chooses the format based on the name of the extension and has a different format for ADF. Rename to .ADL and try again...

Did you use the SD card formatter to format them? Modern versions of windows are known to have issues with formatting SD cards?

The read logic of the disk drive has failed. I tracked down the probable cause of the fault: the 15uF capacitor in the picture had shorted, taking out the inductor next to it. Welcome VectorEyes, and good luck with cleaning the PCB! You may also need to replace some components. The CMOS chip is sensitive to the alkali attack of the battery but you can still get them. We have seen some other repairs on the forum, so share your findings and we can help you along.

This looks clean, and there is no soldering required, but I've made a few of these and in my experience this configuration can give connection issues over time. The IDC connection is ok for a straight flat cable, but with twists and turns in the individual wires it is not so great. Imo soldering on ...
